The Heart of the Role

As a Care Manager, you will lead with purpose - empowering your team and making a lasting impact on the lives of our residents every day. You will support the General Manager in managing and leading the delivery of ongoing assessment, care planning and evaluation and clinical governance.

  • Oversee daily clinical operations ensuring residents receive the best care possible.
  • Support and lead the care home clinical team including coaching, mentoring, staff allocations and delegation, performance management and education.
  • Review clinical incidents, complaints and changes providing solutions to reduce risks.
  • Ensure compliance with AN-ACC assessments.
  • Manage person-centred care planning and evaluation for residents including case conferencing with families and carers.
  • Foster a strong team environment.
  • Prepare and conduct clinical governance audits.
  • Provide insights and innovative solutions for continuous clinical improvement.
  • Oversee medication management in accordance with resident needs and legislative requirements.
  • Liaise with allied health, medical practitioners and other services to provide optimal resident care.
Essential Qualifications and Skills
  • Bachelor of Nursing with current APHRA registration is essential.
  • Post grad qualification in aged care is highly regarded.
  • Strong leadership, communication and interpersonal skills.
  • A person-centred care mindset.
  • A passion for nurturing a strong team culture.
  • Ingrained empathy for our residents.
  • Proven track record in delivering quality clinical care within an aged care or acute setting.
  • Excellent clinical assessment and triaging skills as well as strong care planning capabilities.
  • Proven experience in fostering a reporting culture balanced with effective investigative skills.
  • Understanding of Resident Rights and Aged Care Accreditation Standards.
