Primary Care APC Role

We are seeking a highly skilled Primary Care APC to provide comprehensive, longitudinal clinical and administrative coordination of all care for a specific panel of patients.

  • This role is critical in our ambulatory care setting, where you will work collaboratively with a team of healthcare professionals to deliver exceptional patient care.

The successful candidate will be responsible for providing medical services including physical assessment, treatment, supervision of care plans, and communication with specialists as needed.

Key Responsibilities:
  • Comprehensive health and developmental exams through baseline histories, review of systems, and physical exams.
  • Differentiates normal from abnormal findings.
  • Oversees the monitoring and management of patients with acute and chronic illnesses.
  • Adjusts treatment regimens based on the patient's response to treatment.
  • Ensures coordinated management of patients with certain chronic diseases between primary care team, subspecialty clinicians, and case management.
  • Partners with patients and families to manage care plan.
  • Demonstrates empathy and effective communication skills in patient and family interactions.
  • Collaborates with colleagues in a team-based model of care.
  • Prescribes medications in accordance with Massachusetts statutes and professional guidelines.
  • Reviews incoming clinical data and follows up in a timely manner.
  • Provides consultation regarding hospitalized patients in the panel.
  • Efficiently utilizes electronic medical record to manage practice through effective communications and documentation.
  • Adheres to standards of daily task management, productivity, and quality performance measures.
