Patient Care Coordinator Role
Job DescriptionThe role of a Patient Care Coordinator is crucial in ensuring seamless patient care. This position involves working closely with medical and nursing staff to provide comprehensive care to patients.
The coordinator will be responsible for attending to patients in order of medical urgency, taking a full history and medical examination of new admissions, and arranging investigations as needed.
They will also manage medical conditions under the supervision of consultants, collaborate with multidisciplinary team members to facilitate patient management, and communicate with family and outside medical practitioners regarding patient progress.
Effective documentation of clinical notes throughout each episode of patient care is essential, as well as managing patients and documents with regard to risk management principles.
