Community Outreach Coordinator

We are seeking a dedicated and empathetic professional to provide support to individuals with disabilities and aged care needs.

The role involves delivering person-centered support, promoting independence and well-being in a collaborative environment.

Key Responsibilities:

  • Providing daily living assistance, including personal care and household tasks
  • Developing and implementing individualized support plans to promote client autonomy
  • Collaborating with healthcare professionals to ensure seamless care delivery
  • Maintaining accurate records and reporting changes in client health and wellbeing

Requirements:

  • Minimum Certificate III qualification in Disability or Aged Care, or relevant experience
  • C class driver's license and registered vehicle
  • Current National Police Clearance and NDIS Worker Screening
  • First Aid Certificate and Working with Children's Check
  • Flexible approach to hours and ability to work irregular shifts
