Community Care Liaison Opportunity

Make a meaningful difference in the lives of indigenous communities by facilitating access to care and support services.

This role offers a unique chance to live and work in a stunning regional setting, surrounded by beautiful rainforests, picturesque gorges, and diverse wildlife.

The successful candidate will play a vital part in case managing clients accessing community care programs, including Home Care Packages (HCP), Commonwealth Home Support Programme (CHSP), and National Disability Insurance Scheme (NDIS) initiatives. Working collaboratively with a dedicated team, they will engage with local communities, drive positive change, and contribute to the development of sustainable solutions.

Key Responsibilities:
  • Foster relationships with members of the local community to ensure equitable access to care and support services.
  • Develop and maintain effective communication channels with stakeholders to promote program goals and objectives.
  • Work closely with the Regional Manager to achieve program outcomes and advance community care initiatives.
Requirements:
  • Previous experience in community service coordination or a related field.
  • A strong understanding of social determinants impacting indigenous populations and a passion for working in this area.
  • Ability to lead a small team and collaborate effectively with colleagues.
  • Strong knowledge of legislative requirements within community healthcare.
