Job Summary
The Enriched Living Care Team Leader plays a vital role in ensuring the overall comfort, safety, and well-being of our residents. As a leader, you will be responsible for assessing individual resident needs, supervising staff, and coordinating care delivery. Your excellent communication skills and ability to work collaboratively with colleagues will enable you to provide exceptional service to our residents and their families.
Responsibilities

- Supervise and direct activities of Certified Home Health Aides, including personal care assistance, social and leisure activities, and emotional support to residents.">
- Monitor delivery of resident care to ensure high-quality customer service and adherence to policies and procedures.">
- Perform any necessary functions of personal care, maintain accurate records, and communicate effectively with families, residents, and medical management teams.">
- Supervise and provide care to residents who are ill, reporting changes in condition promptly and maintaining accurate documentation.">
- Manage resident medications and supplies, ordering reorders as needed, and maintaining accurate records for narcotic substances.">
- Prepare physician consult sheets for medical appointments, facilitate completion of documentation, and promote staff development through training and professional development programs.">
- Maintain a healthy and collaborative team environment, promoting open communication, and resolving conflicts effectively.">
- Monitor daily staffing and assign adequate staff coverage for resident care, ensuring the smooth operation and delivery of all services.">
- Understand and verbalize the emergency preparedness plan, taking an active role in responding to events.">
Requirements

- A current New York State license in good standing as a Licensed Practical Nurse or equivalent experience.">
- A minimum of 2 years' supervisory experience in a related field, preferably in a union environment.">
- A high school diploma or higher education required, with familiarity with medical transcriptions and proficiency in MS Office.">
- Excellent interpersonal skills, strong leadership abilities, and the capacity to motivate others.">
- A flexible schedule and the ability to work efficiently under time constraints.">
- Competitive salary range: $32-$33 per hour, commensurate with experience.">
