Oncology Social Worker

Role Overview
This role involves providing psychosocial assessments and complex discharge planning for patients in two inpatient oncology wards with a large elderly population.
The ideal candidate will work closely with medical teams to deliver patient-centered care in a supportive, part-time capacity.
Key responsibilities include delivering high-quality psychosocial assessments, leading complex discharge planning with multidisciplinary teams, and providing support to patients, families, and carers.
Requirements:
- Professional qualification in Social Work
- Current registration or eligibility for AASW membership
- Strong skills in discharge planning and assessments
- Full vaccination against COVID-19, including booster shots
Patient-Centred Care
Delivering high-quality patient-centred care is at the heart of this role. You will work closely with medical teams to ensure seamless coordination of care and provide emotional support to patients and their families.
Discharge Planning and Assessments
You will be responsible for conducting comprehensive psychosocial assessments and developing tailored discharge plans that meet the unique needs of each patient.
