We are seeking a highly skilled and experienced Social Worker to join our team at Northern Beaches Hospital, a leading healthcare provider in Australia. As a Social Worker, you will play a critical role in providing high-quality clinical care to patients and their families, working collaboratively with medical officers and other healthcare professionals to deliver evidence-based treatment and support.
Key Responsibilities- Provide comprehensive social work assessments and interventions to patients in acute care settings, including surgical, medical, critical care, emergency, maternity, paediatrics, and aged care.
- Develop and implement individualized care plans to address patients' social, emotional, and psychological needs.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
- Participate in social work weekend on-call crisis roster to provide 24/7 support to patients and families.
- Master's degree in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW).
- Proven experience working in an acute healthcare setting, with a strong understanding of social work principles and practices.
- Excellent communication, interpersonal, and counseling skills, with the ability to work effectively with diverse patient populations.
- Ability to prioritize and manage workload demands, meet deadlines, and complete activities within scope of practice.
