Your Opportunity
Tired of working on the frontline? Looking to join a team coordinating care and services to support patients in their own home? We have an opportunity available to work within the SA Community Care (SACC) and the My Home Hospital (MHH) program.
Using your clinical skills, you will ensure a comprehensive client centred management plan is achieved for our clients together with:
1. Conducting clients initial phone assessment, care planning and goal setting
2. Coordination of in-home services to support clients receiving care at home
3. Building and maintaining effective relationships with a variety of different stakeholders
4. Monitoring patient progress and updating care plans as appropriate
5. Liaison with healthcare professionals and specialists
6. Discharge planning including referrals to long term services
7. Provision of telephone support, education, and advice
Our Care Coordinators work a rotating 7-day roster between the hours of 7am & 8pm. You will be rostered to work alternate weekends in the comfort of your own home. Please note that this role is based in Adelaide (South Australia) and we offer great flexible working options to achieve a better work-life balance for you & your family.
We have two part time 0.6 FTE fixed term parental leave positions for 12 months, this could provide you the opportunity to experience virtual nursing and a great career development opportunity.
What will set you apart from the rest?
An experienced clinician, you are customer focused and driving to deliver best practice outcomes across our programs. Degree qualified as a RN, you bring a clinical background in an acute hospital setting.
- 3-5 years' experience acute hospital experience as a Registered Nurse with unrestricted AHPRA registration
- Recent experience in care coordination (highly desirable)
- Strong customer service orientation and relationship management skills
- Excellent clinical reasoning skills
- Ability to work in a fast-paced environment.
- Proficient computer skills to enable fast and accurate data entry into patient electronic medical records.
