Founded in 1979, Carrie's Place Domestic Violence and Homelessness Service is a committed team of professionals dedicated to raising awareness of domestic violence within the community. As a not-for-profit organisation, they offer a wide range of culturally sensitive services to individuals affected by domestic violence and homelessness.
Their mission is to empower families in the Maitland and Lower Hunter regions by helping them find safe, stable housing and live free from abuse. Carrie's Place works closely with local communities to address domestic violence and homelessness, providing holistic support that fosters capacity-building, inclusion, and resilience.
The passionate team collaborates to offer a variety of programs focusing on equipping clients with the skills, knowledge, and confidence to maintain healthy relationships and achieve greater independence.
About the role
As the Supporting Outreach Healthcare – Integrated Coordinator, you will be at the heart of a dynamic, multi-agency effort to support women and children experiencing domestic and family violence. This role is about more than just coordinating services; it's about shaping pathways to safety, health, and well-being for vulnerable families.
You will coordinate a team of health professionals from diverse disciplines, ensuring seamless integration of health care and social support services for those in crisis accommodation. While your direct contact with families is limited, you will make a lasting impact by driving collaboration across sectors, ensuring that every woman and child receives the care they need, when they need it. The role is offered on a two-year contract, supporting the coordination of a vital community-focused service.
Key Responsibilities:
- Oversee daily program operations and ensure effective service delivery across all sites, supporting a high standard of care.
- Lead Care Collaboration meetings and stakeholder engagement to foster strong multi-agency partnerships and integrated, trauma-informed support.
- Coordinate clinicians and refuge services, managing referrals, clinic scheduling, and systems use (e.g. JustCare) to ensure seamless healthcare access.
- Support the DFV workforce and Outreach team, including appointment attendance, on-the-spot debriefing, and response to critical incidents.
- Champion client advocacy and compliance, ensuring timely, holistic care aligned with legislation, policies, and best practice standards.
- Contribute to program evaluation and reporting, including data collection, funding reports, and national pilot evaluation activities.
- Participate in codesign and communities of practice, promoting strengths-based collaboration and continuous improvement across services.

About you
We're seeking an experienced professional from the human services or healthcare sector who thrives in collaborative, multidisciplinary environments. You bring strong coordination and stakeholder engagement skills, along with a proactive and compassionate approach to program delivery.
Ideally, you will have:
- Proven experience coordinating multi-agency responses within a human services or healthcare environment.
- Strong leadership capabilities, including overseeing day-to-day operations, supporting frontline staff, and chairing collaborative meetings.
- Exceptional communication skills, with strengths in negotiation, advocacy, conflict resolution, and the ability to engage effectively with diverse stakeholders.
- A genuine commitment to advocating for vulnerable families and improving service outcomes.
- High-level organisational and administrative skills, including drafting complex documents and managing information across multiple data systems.
- Sound knowledge of domestic, family, and sexual violence, including its impacts on both adults and children, is highly desirable.
- Demonstrated ability to implement trauma-informed and client-centred practices, and to support others in working this way is highly desirable.
Please note that this role requires a current NSW Driver's Licence, National Police Check and NSW Working with Children Check, or willingness to obtain.
