We’re looking for our next Support Coordinator to join the team of Life Changers. Could it be you?

The Role

As a Support Coordinator, you’ll be the go-to person helping participants bring their NDIS and Care Plans to life. You’ll connect them with the right services, build their independence, and keep everything running smoothly because when the plan works, the person thrives.

- Connecting participants with the right mix of supports (informal, mainstream, and funded)
- Advocating fiercely for participant needs and goals
- Navigating the NDIS with confidence
- Building capacity and independence along the way
- Creating strong, lasting relationships with participants, families, and providers
- Keeping on top of reporting, compliance, and documentation

About you

You’re passionate about making a difference and have experience in the field. You will bring:

- 2+ years’ experience in disability, support coordination, or community services
- Tertiary qualifications in disability, community services, or related fields
- Strong knowledge of NDIS and person-centred practice
- Strong communicator and natural relationship builder
- Organised, self-motivated, and able to juggle competing priorities
- Empathetic, solutions-focused, and outcome-driven
- Current Victorian Driver’s Licence and reliable vehicle
- Safety Screening Checks and mandatory training (including NDIS Worker Screening, Police Check, WWCC, First Aid & CPR)
