At the moment, the team you will be joining is currently known as the Specialist Adult Pathway Team (SAPT) but it will be changing its name to Preparing for Adulthood team once successful recruitment of key posts has been completed. This will then allow us to fully begin to meet one of our aspirations which is the creation of a Preparing for Adulthood Team.

SAPT sits alongside the Community Learning Disability Team as part of the All Age Disability Team. The role of SAPT is to work with young people aged 14-25 years and their families with a focus on preparing them for adulthood. This involves close liaison with the Children with Disabilities service and other Children’s Social Care teams. In preparing the young person for adulthood, the team undertakes holistic strength-based assessment and develop personalised support plans to meet the young person/'s needs and outcomes, in preparation for them turning 18.

Required Skills and Experience for the Role

- Change management skills are vital to ensure that changes are embraced positively by everyone in the team, whilst fostering a creative ethos to guarantee vulnerable adults are supported with care that is individual to them.
- As part of this role, you will be expected to positively manage team change, growth, support the development of new pathways, support the improvement of current pathways and identify gaps in services with the support of the Team Manager and the All Age Disability Service Manager.

As a service we pride ourselves on supporting each other. We are approachable, supportive and celebrate our successes as a service and not as individuals. We are an experienced service; we all pull together to support each other and share our expertise.

If you would like more information about the Service, Team or Position please contact the Lauren James-Kinder, Team Manager via email to arrange an informal chat Lauren.james-kinder@kirklees.gov.uk
