About the Role As a Nurse Practitioner (NP) in our Palliative Care Service, you will bring high‑level clinical expertise to support complex and evolving care needs. You’ll work across residential aged care, hospital in the home (HiTH), and community settings, including ambulatory clinics. In this autonomous yet collaborative role, you will: - Provide expert clinical assessments and implement advanced care plans - Lead initiatives in evidence‑based practice and clinical service planning - Actively contribute to quality improvement and mentoring of staff - Collaborate with the Community Palliative Care Team (CPCT) to enhance patient outcomes across care environments What You’ll Bring - Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse who is endorsed to practice as a Nurse Practitioner by the Nursing and Midwifery Board of Australia. - Demonstrated ability to work independently and in the absence of a Medical Practitioner to provide quality care together with the capacity to work collaboratively as part of a multidisciplinary team. - Demonstrated high level knowledge and advanced clinical nursing skills including comprehensive assessment, interpretation of diagnostics, implementation of treatment modalities and relevant referral skills applied to a broad range of challenging, complex and changing situations. - Demonstrated understanding of the palliative care delivery system and ability to lead and implement changes and develop direction in clinical practice by the initiation, utilisation and review of evidence‑based research and ability to undertake clinical service planning. - Proven high‑level knowledge and expertise in complex client centred consultancy, practice development activities including mentoring, education, active participation in policy development and quality improvement. - Demonstrated advanced clinical leadership role, negotiation, liaison, interpersonal and problem‑solving skills across the palliative care service that includes acute, sub‑acute and aged care sectors. - Current NSW Class C Unrestricted Driver’s License. - Ability to work within the Mission and Values of Calvary Health Care Kogarah.
