What you will do:
– Employ a trauma-informed, person-centred approach to support those experiencing multiple complex needs.
– Cooperate with a multi-disciplinary team to provide efficient and effective service provision.
– Fulfil statutory duties under the Care Act 2014 and Mental Capacity Act 2005 to ensure exceptional care for all clients.
– Contribute to identifying blocks and barriers within the system, using innovative thinking to enhance service delivery.
– Engage in direct client work, conducting thorough assessments and delivering tailored interventions to support positive outcomes.
