HammondCare provides care primarily through hospitals, residential care homes and home-based care as well as services for dementia support, community palliative care, rehabilitation, positive aging and older persons’ mental health.**About the role**Reporting to the Head of Pastoral Care, the Pastoral Care Coordinator provides pastoral care, leadership and support to clients, their families, team members and pastoral care volunteers. The role supports clients, who are in HammondCare’s palliative care service, by visiting them in their own homes across the Northshore.The role requires access to a car (mileage reimbursed), use of a pool vehicle may be available.
