About the Role
We are currently seeking a dedicated and experienced Care Manager to lead our passionate team in the Northern Rivers region. Reporting to the General Manager and working closely with a collaborative leadership team, you will be responsible for overseeing clinical care services, supporting our clients, and mentoring the team of Care Partners.

Key Responsibilities
- Lead and support the Northern Rivers care team to deliver person-centred care in line with Aged Care and NDIS Quality Standards.
- Manage clinical governance, including assessments, care planning, incident management, and internal audits.
- Mentor and guide clinical and care coordination staff, fostering a culture of continuous learning and excellence.
- Work collaboratively with Directors, the General Manager, and Client Services Managers to meet strategic and operational goals.
- Ensure compliance and uphold the highest standards of clinical care delivery.
- Champion client independence and wellbeing by supporting them to live life on their terms.
About You
You’re an experienced, compassionate community clinical leader who:
- Holds current AHPRA registration as a Registered Nurse (without restrictions).
- Has strong experience in community care, including complex clinical needs, medication management, catheterisation, and diabetes care.
- Understands compliance requirements within the Aged Care and NDIS frameworks.
- Has a track record of leading high-performing teams and achieving KPIs related to care quality and outcomes.
- Demonstrates excellent communication and relationship-building skills.
- Is proactive, solution-oriented, and confident in decision-making.
- Has previous experience in home care, disability support, or community health (highly regarded).
- Is tech-savvy with strong documentation and digital literacy skills.
