About the Role

Churches of Christ are seeking a caring, reliable, and clinically skilled Registered Nurse (Level 2) to take on the role of Clinical Care Partner within our dynamic Home Care team across Bundaberg and Hervey Bay.

Reporting directly to the Service Manager and working in close partnership, this role plays a critical part in maintaining clinical oversight and governance across both teams. Together, you/'ll ensure safe, high-quality, person-centred care and continuous improvement in practice.

This is a hands-on clinical leadership role that combines oversight, mentoring, and collaboration. You/'ll guide and support Care Partners and Home Care Workers to deliver safe, high-quality, person-centred care aligned with the upcoming Support at Home Program.

Key Responsibilities

- Assess Client Needs: Conduct comprehensive and ongoing assessments to identify physical, emotional, and social needs, tailoring care plans with clients and families.

- Care Planning: Develop, review, and maintain person-centred care plans aligned with client goals, clinical best practice, and funding requirements.

- Coordinate Services: Liaise with clients, families, and external providers to ensure timely, appropriate, and high-quality supports.

- Clinical Oversight: Provide expert nursing guidance to Care Partners and Home Care Workers, ensuring clinical safety, compliance, and best-practice care delivery.

- Advocacy & Education: Empower clients and staff through education and advocacy, promoting understanding of rights, funding, and care choices.

- Compliance & Safety: Maintain accurate documentation, uphold aged-care standards, and proactively manage clinical and operational risks.

Who We/'re Looking For

You are a compassionate, organised clinician who thrives in a community-based environment, balancing clinical expertise with supportive leadership. You enjoy mentoring others, problem-solving, and ensuring the highest standards of care across a growing service.

Our Ideal Candidate Will Have

- Current AHPRA registration as a Registered Nurse (Level 2) - essential

- Demonstrated experience in aged care, home care, or community health

- Strong communication, organisational, and critical-thinking skills

- Current First Aid and CPR certification (or willingness to obtain)

- Current National Police Check (or willingness to obtain)

- Current National Personal Insolvency Check (or willingness to obtain)

- A current driver/'s licence and reliable, comprehensively insured vehicle ($0.96/km reimbursement)

- Australian work rights (we are not able to offer visa sponsorship)
