Overview
Registered Nurse - Care Coordinator location: Bundaberg. Working within Gracehaven, our 115 bed Residential Aged Care home, the role provides advanced clinical support, leadership and coordination of clinical care to staff and residents.
The Care Coordinator is a pivotal role tasked with coordinating care, liaising with external providers, and ensuring quality clinical care across the home.
Responsibilities
- Coordinating and liaising with external providers
- Overseeing and coordinating the completion of baseline and focus assessments
- Developing and reviewing care plans
- Coordinating the collection of accurate clinical and care data for timely submission and reporting
- Providing clinical leadership and supporting RNs in developing clinical leadership skills
- Assisting the Clinical Manager with follow up and oversight in quality clinical care
About You
Are you somebody who is passionate about delivering quality aged care services to the local community?
Essential:
- Passionate about Aged Care and providing safe supportive care to residents
- Current AHPRA registration as a Registered Nurse (RN)
- Minimum 3 years clinical experience in the Aged Care Sector
- Current driver's licence
- Currently holds, or willing to obtain a current Police Check and Blue Card
- Hold valid Australian Work Rights – We are not currently able to sponsor
Desirable:
- Post-graduate qualifications in Geriatrics, Dementia or other aged care specialisation
- Currently hold, or willing to obtain a current First Aid and CPR Certificate
- Currently hold, or willing to obtain a current NDIS check
