Fremantle Hospital Mental Health Service is seeking a dedicated and experienced Senior Social Worker to join our dynamic Transitional Care Team.

We are committed to providing high-quality social work services to individuals experiencing mental health and homelessness related issues, their families/significant others from a trauma informed and recovery orientated perspective. This is a recruitment pool for Permanent Full Time/Part Time, Fixed Term Full Time/Part Time and Casual appointments with the possibility of extensions and/or permanency.

Suitable applicants will be placed in a pool from which appointments may be made when similar vacancies occur. Appointments may be made from this pool until 30/09/2025.

- A detailed current Curriculum Vitae
- The names and contact details of two (2) current referees who can attest to your suitability for this role.
- A brief covering letter not exceeding three (3) pages, addressing the below selection criteria (Essential 2, 3, 6 and Desirable 2, 4) as outlined in the attached JDF.

Demonstrated extensive relevant clinical experience and knowledge and advanced skills in appropriate assessment, treatment, and evaluation within the Social Work specialty area of Mental Health.

Demonstrated ability to plan, develop, coordinate, implement and evaluate Social Work services including application of quality improvement principles and practices.

Current knowledge of legislative obligations for Equal Opportunity, Disability Services and Work Health and Safety, and how these impact on employment and service delivery.

Demonstrated ability to work effectively in a multidisciplinary team setting.

Previous experience in community development and/or homelessness sector.
