What you will do:
As a Social Worker in Mental Health, your key responsibilities will encompass coordinating care for complex cases under the Care Programme Approach (CPA). You will develop collaborative care plans alongside service users, ensuring their voices are heard. Conduct comprehensive risk assessments and manage safeguarding effectively. Engage in holistic mental health and social care needs assessments, including conducting reviews under the Care Act. Work closely with carers and wider support networks, fostering partnership working with both statutory and voluntary organisations. Liaise with inpatient units and maintain diligent record keeping and report writing. Your role demands experience working with adults experiencing psychosis and complex needs, along with Care Act assessments. You will need excellent time management and organisational skills, strong interpersonal abilities to connect with a diverse clientele, and must be a cooperative team player. A thorough understanding of relevant legislation such as the Safeguarding, Mental Health Act, Care Act, and Mental Capacity Act is essential.
