Digital & Transformation Lead

The postholder will lead digital transformation and service improvement across the Primary Care Network (PCN). They will use a data-driven approach to analyse service models, identify challenges, prioritise improvements, and monitor the impact of changes.
Key responsibilities include:
Driving digital transformation across ten practices, improving access, efficiency, and patient outcomes.
Using data tools (e.g., RAIDR, QOF) to identify health inequalities, streamline pathways, and support performance improvement.
Promoting NHS App uptake, standardising communication, delivering digital training, and exploring AI solutions.
Supporting data-informed decision-making, establishing effective data-sharing mechanisms, and tracking progress using analytics.
Applying project management techniques (PPM) to plan, deliver, and evaluate transformation initiatives.
Analysing population health data to address inequities and guide the PCNs improvement plan.
Reporting regularly to PCN leadership on outcomes, challenges, and opportunities for continuous improvement.
In essence, the role combines data analysis, digital innovation, and strategic leadership to drive continuous improvement and reduce health disparities across the PCN.

Main duties of the job
- Develop and implement strategic plans to optimise clinical systems and strengthen the PCNs digital infrastructure.
- Lead transformation projects from design to delivery, improving patient care, access, and operational efficiency.
- Oversee data management and analytics to inform decision-making, ensure GDPR compliance, and measure impact.
- Provide regular progress reports to PCN leadership and practices, highlighting capacity, progress, and challenges.
- Drive quality improvement initiatives using data-driven insights and stakeholder feedback.
- Use RAIDR, QOF, and other datasets to identify health inequalities, streamline pathways, and improve outcomes.
- Deliver projects supporting population health management, access to care, and system efficiency.
- Lead NHS App promotion across practices to increase uptake and patient engagement.
- Explore and evaluate AI solutions to enhance PCN operations and patient outcomes.
- Cascade learning from digital meetings across all practices to save time and resources.
- Develop and deliver a PCN-wide digital training plan to build staff capability.
- Standardise SMS templates and communications in line with ICB directives to reduce costs.
- Align PCN digital strategy with national and local priorities, supporting integrated working and improved patient access.

Eden Primary Care Network (PCN) and its ten member GP Practices are working together to improve the health and well-being of the Eden population and provide high quality, integrated services across the community.
Job responsibilities
The postholder will work to understand current service models, identifychallenges, agree priorities for improvement, and support the delivery of theseimprovements across the PCN.
Taking a data-driven approach, they will analyse available evidence tounderstand operational challenges, identify potential solutions, and track theimpact of changes. They will effectively present and communicate data topractices and PCN staff at all levels, enhancing understanding of operationaland clinical issues.
The postholder will lead the PCNs digital and transformation agenda,optimising clinical systems and infrastructure to improve access, efficiency,and patient outcomes across ten practices. They will use data sources such asRAIDR and QOF to identify health inequalities, streamline pathways, and supportperformance improvement.
Responsibilities include driving NHS App uptake, standardisingcommunications, delivering digital training, exploring AI solutions, sharinglearning from digital meetings, and ensuring alignment with ICB priorities.
The role will support a focus on data-informed decision-making, securinganalytical resources as required, and developing appropriate data andinformation-sharing mechanisms across the PCN. They will use data to track andassess the impact of operational changes over time and identify trends toinform continuous improvement.
Using Project Portfolio Management (PPM) techniques, the postholder willfocus on delivery of changes, analyse impacts, and provide feedback to staff onboth positive and negative outcomes. They will advise on innovativeopportunities and support practices to maximise benefits from digital andservice innovation through sharing learning and best practice.
The postholder will analyse population health data toidentify inequities and inform the development and delivery of an agreed PCNplan aimed at improving health outcomes and reducing disparities across thecommunity. They will provide regular feedback to PCN leadership and memberpractices on the progress of initiatives, capacity for change, and emergingissues, ensuring plans are adapted in response to evidence and operationalneeds.

Qualifications
- GCSE Maths & English or equivalent
- Dealing with the Public
- Use of office equipment
- Degree or equivalent level of knowledge through experience
- Able to use own initiative & work autonomously

Skills
- Excellent understanding of data management principles and the ability to leverage healthcare data for decision-making.
- Strategic thinker with the ability to develop and implement long-term digital strategies aligned with organisational goals.
- Excellent communication skills, both written and verbal, with the ability to articulate complex technical concepts to non-technical audiences.
Knowledge
- Previous experience in general practice or primary care
- Project management qualification (e.g. PRINCE2) or equivalent experience of managing projects
- Use of EMIS
- Experience supporting senior leaders and working with external partners
- Strong data handling and reporting skills
- Excellent interpersonal and communication skills
- Awareness of principles of:
