About the role
This position sits within the Outreach Cluster, which aims to improve the health and wellbeing of people who use alcohol and other drugs.
The Case Worker works with other team members to provide structured support, identify needs and build goal directed care plans. The Case Worker makes referrals to other services, provides regular support to clients, works alongside other members of the team, and holds a case load to provide short to medium term care to clients who meet the criteria for case work by the City Street Health team.

Responsibilities
- Provide harm reduction education, advocacy, referral, needles/syringes and other injecting and disposal equipment to clients to support safer using and disposal through Outreach
- Using safe strategies, work with intoxicated consumers and assess overdose risk
- Provide secondary consultations to other cohealth staff as required and assist in developing strategies to effectively manage clients with complex needs
Qualifications and requirements
- Minimum Certificate IV (Alcohol and Other Drugs) and/or tertiary studies in Community Development, Social Work etc
- Lived experience will be highly regarded
- Sound knowledge of the health and social issues and service sector for people who use substances and sleep rough
- A commitment to the principles of harm reduction and recovery

All appointments are made subject to a satisfactory Nationally Coordinated Criminal History Check (NCCHC), a Working With Children Check (WWCC), complete a Pre-Employment Declaration Form, evidence of legal rights to work in Australia, and evidence of vaccination.
Successful applicants will be required to provide evidence of immunisation assessment including Influenza within 12 weeks of commencement. This is in accordance with the Victorian Minister of Health Mandatory Vaccination Orders and legislated requirement for influenza vaccination.
