Gold Coast City, Australia NHS Full time

We are seeking an experienced, qualified and motivated Advanced Nurse Practitioner to join our friendly GP Practice at Christiana Hartley Medical Practice.

You will work autonomously to provide high-quality care across both acute and long-term conditions. Previous experience within general practice is desirable. The post holder will work well as part of a team, contribute to shared goals and is eager to grow professionally within a progressive patient focused setting.

For the right candidtae we are open to a full or part time position, with an attractive salary.

Main duties of the job

You will play a vital role in offering expert, clinical care, managing both acute and chronic conditions. You will work independently, within your scope of practice, supported by a collaborative and supportive multidisciplinary team, including GP's, ANP's, pharmacists and the wider team. There a regular team meetings with supportive and approachable clinical leadership.

You will have a mix of face to face and telephone consultations. Responsibilities include:

- Assessing, diagnosing and managing undifferentiated and undiagnosed conditions.
- Prescribing and reviewing medicines.
- Requesting and interpreting investigations.
- Triaging and supporting urgent clinical queries.
- Home visits, when required.
- Personalised care planning.
- Adhere to practice policies and procedures.
