Main duties of the job Our Support Workers are the unsung heroes, providing crucial assistance to customers in the comfort of their homes post-hospital discharge. Collaborating with dedicated occupational therapists, we co-develop personalised reablement plans to help customers achieve their goals at home. The team operates on a rotating schedule, available 7 am to 10 pm, 365 days a year. As a Senior Support Worker, you will monitor, support and supervise our Support Workers to ensure the highest standard of service for our customers. We welcome candidates with either an NVQ Level 3 or equivalent substantial experience in a related field and those interested in pursuing a relevant qualification. Job responsibilities Lead a team of reablement workers within a defined geographic area. Provide ongoing monitoring, support and supervision to reablement workers and ensure services are delivered to high standards with quality assurance, compliant with legal requirements and within an effective performance management and budgetary framework. Work closely with occupational therapy staff to create person-centred and CQC-compliant reablement support plans. Attend the customer/'s first visit to introduce the service and support understanding of the reablement approach. Undertake risk assessments and encourage positive risk-taking, including environmental, medication and mobility risks, recording actions and agreed strategies. Collaborate with partners including health teams, care homes, care agencies and hospitals. Understand the discharge process and management of cases transferring into reablement; gather relevant information and ensure safe management of risks for customers and staff. Assist in providing safe care in line with regulations and quality standards. Lead out-of-hours cover as required and ensure procedures are followed, with knowledge of CQC regulations and inspection processes. Complete, distribute and monitor paperwork required by CQC registration, including medication administration records, ensuring paperwork is thorough and ready for inspection. Drive effective communication between professionals and customers and their families to reduce duplication and ensure coordinated care. Provide customers with information about the reablement service, including expectations and how to raise concerns. Record reablement contacts accurately and confidentially, capturing actions, events and decisions in line with case management principles. Monitor progress toward reablement goals and provide feedback to occupational therapists and the case manager. Operate under the direction of the Team Leader/Registered Manager to ensure smooth and efficient service delivery. Demonstrate good organisational skills and a flexible approach to support the team through changes. Report changes in reablement support plan visits promptly, and liaise with the allocator/co-ordinator to match workers with customers and assist in electronic rostering when needed. Provide information, support and guidance to reablement workers to enable safe and effective delivery of reablement support as per plans. Ensure services are delivered according to identified customer needs. Handle on-call/emergency issues, cover visits as needed, and provide timely feedback to the team on after-hours issues. As a Senior reablement worker, exercise timely and authoritative decision-making. Participate in transfers of customers requiring further support to other internal teams or providers. Engage in regular supervision sessions and ensure agreed actions are completed on time. Monitor and support a small group of reablement workers to ensure performance and behaviours meet required standards. Promote continuing professional development, reflective practice and evidence-based learning, and report concerns to the Team Leader/Registered Manager. Participate in staff appraisals and development where applicable. Under the Team Leader/Registered Manager, provide induction for new staff as needed, including mentoring, shadowing and in-field training. Monitor the delivery of reablement support in line with service policy, including reviewing daily communications, observing visits, providing one-to-one supervision, conducting spot-checks of practice and auditing files. Maintain safeguarding awareness in line with Wiltshire policy and escalate referrals as required. Be an ambassador for the Council and department, representing the Council positively and professionally in meetings with customers, partners and agencies. Be prepared to respond to civil emergencies under instruction and lead improvements to service practice. Person Specification Qualifications - NVQ Level 4 or equivalent significant experience in a relevant field (or a commitment to gaining it) - Experience of supporting adults within a care environment - HSE recognised First Aid at Work qualification or willingness to achieve - Experience in supervising others or ability to demonstrate required skills - Up-to-date knowledge of relevant legislation and safeguarding of adults - Experience and current knowledge of moving and handling procedures - Good interpersonal skills and ability to communicate effectively with people at different levels - Good organisational skills and ability to prioritise tasks - Ability to use electronic social care records - Physically fit to perform all job requirements - Ability to work in a person-centred way to meet individual outcomes - Fully fluent in spoken and written English - Experience of working in a reablement service and in a CQC registered service - Understanding of the Care Act 2014; Health and Social Care Act 2008 - Able to work as part of a team and provide/implement instructions - Qualification as a trainer (preferred) Experience - High level of relevant experience and practical knowledge - Advanced understanding of procedures and practices relevant to the service area - Experience working with clients with complex needs - Strong ICT skills including Microsoft applications and specialist systems - Proven ability to carry out client risk assessments - Ability to guide clients to access services they are entitled to - Ability to build trust with clients and colleagues - Ability to interpret situations and deliver interventions to achieve outcomes - Proven ability to deliver training Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such a disclosure check will be required. £30,559 to £32,654 a year plus 10% unsocial hours for qualifying shifts #J-18808-Ljbffr
