Salary: $70,511 - $90,659 pa (Pro Rata + Super + Salary Packaging)

AQF Level 7 qualification or above and current and full registration with AHPRA (for psychologists) OR eligibility for membership with the AASW (for social workers) or registered with ACA or PACFA (for Counsellors) and a minimum of 3 years' recent experience.

Job Status

Part time, 0.4 FTE

Responsibilities

- Provide focused cardiac health support to people with heart disease, including Congestive Heart Failure, Ischaemic Heart Disease, Coronary Artery Disease and Heart Attack.

- Deliver holistic support of heart disease health services and engage services for people in FocusOne Health's Murray Mallee catchment, including individual case management with a multidisciplinary team and outreach community education-based approaches.

- Collaborate as part of a multidisciplinary team to support patient care and engagement with health services.

Qualifications

- Eligible disciplines include Social Work, Psychology and Counselling.

- Social workers: AQF Level 7 qualification or above with current and full registration with AHPRA, or eligibility for AASW membership; psychologists: current registration with AHPRA; counsellors: registration with ACA or PACFA; minimum of 3 years' recent experience.
