Work Location: Prince of Wales Hospital – Randwick (Community AIN role within TACP, a multi-disciplinary home-based rehabilitation program for older adults)

This AIN role operates in the Community setting. You will be part of a multi-disciplinary team including Physiotherapists, Occupational Therapists, Social Workers, Community Aides, a Clinical Nurse Consultant and Registered Nurses. You will visit clients discharged from hospital who are within the Transitional Aged Care Program for up to 12 weeks.

Travel and requirements: Travel in the Randwick - Botany Local Government Area; use Hospital fleet vehicles. A current NSW Driver’s Licence and vaccinations as per NSW Health policy are required. Work days are Monday – Friday.

Where you'll be working

This role is in the Community setting, based on the Prince of Wales Hospital Campus,
Randwick. The AIN is part of the TACP multidisciplinary team, including Physiotherapists, Occupational Therapists, Social Workers, Dietitian, Registered Nurses and an Administration Assistant. Travel is in the Randwick – Botany area using hospital fleet vehicles. A NSW Drivers Licence and all vaccinations per NSW Health policy are required.

TACP provides post-acute, home-based rehabilitation for up to 12 weeks for clients over 65 (or 50 for Aboriginal people) living in the Randwick – Botany LGA. There are about 32 clients on TACP at any time; assessments and handovers occur as clients are discharged from hospital.

What you'll be doing

- Provide direct care as part of the TACP, assisting with personal and domestic activities of daily living, function and mobility, community access and client transport; supervised by the Registered Nurse and the multi-disciplinary team
- Contribute to therapeutic interventions to improve physical function and maintain independence, delivered in clients’ homes
- Support the nursing team in delivering nursing care in the community as directed by the Registered Nurse
- Assist Registered and Enrolled Nurses with patient care interventions
- Collect and maintain accurate healthcare information and documentation
- Communicate effectively with patients and the health care team in line with TACP protocols
