Job summary Early Supportive Discharge (ESD) team at Salisbury Hospital aims to enable patients to return home as soon as medically fit, working alongside Wiltshire Council’s re‑ablement team. The service operates a "discharge to assess" model, facilitating discharge plans at the hospital and completing assessments in patients’ homes to ensure the required care and therapy for independent recovery. As a member of the ESD team you will communicate and coordinate with multidisciplinary colleagues, assess and review patients, and may provide care and exercise plans within patients’ homes. A 7‑day service from 08:00–18:00 requires flexible working patterns; the standard week is 08:00–16:00. A full driving licence and access to a car are essential. Responsibilities You will treat and support patients identified for the ESD team, working autonomously with the support of the band 6 team and wider staff. Key duties include: - Setting treatment goals, monitoring progress and encouraging independence with activities of daily living. - Adapting treatment and rehab plans as necessary. - Supporting the daily running of the ESD team and contributing to a positive team ethos. - Demonstrating commitment to high‑standard care and aligning with the trust’s values and the ESD Team Behavioural Charter. Qualifications and Experience - HCPC registration. - Diploma or degree in Occupational Therapy or Physiotherapy. - Experience working in the NHS (including student placements) – desirable. - Evidence of altruistic extracurricular activities – desirable. Additional Requirements - Linking placements to application. - Desire to work at Salisbury NHS Foundation Trust.
