- Full Time (1.0 FTE), Maximum Term role until 16 February 2027 (Parental leave cover, with possibility of extension)
- Band 4: $89,184.51 - $111,200.77 + 12% superannuation
- Generous NFP salary packaging options up to $18,550.
- Location - 737 Bourke Street, Docklands Victoria 3008, Close to Southern Cross Train Station, Onsite or Hybrid; WFH (2-3 days)

Short description about the role
Reporting to the Manager, Children, Young People and Families, the Coordinator, Youth in Education provides operational oversight for State-funded health commissioning and project activities for youth in the education system. The Coordinator is responsible for the planning, implementation, monitoring and evaluation of those services within the purview of NWMPHN. They provide leadership through program development activities, coordination and networking and capacity building.

We are North Western Melbourne Primary Health Network (NWMPHN) - Victoria's largest independent, locally governed and run, not-for-profit PHN, dedicated to improving primary health care in Melbourne's northern, western suburbs and inner-city areas. We help people stay well by commissioning health services to deliver better and more accessible primary care.

- Increasing the efficiency and effectiveness of services for community members, particularly those at risk of poor health outcomes
- Improving coordination of care to ensure people receive the right care in the right place, at the right time.

- Working and building relationships with a wide range of commissioned services and key stakeholders to effectively engage, plan, coordinate, implement and evaluate activities
- Lead and apply best-practice project management methodologies to achieve deliverables and performance targets in a timely manner
- Collaborate with other NWMPHN programs to ensure commissioning contracts, plans and reporting activities are accurate and maintained to meet internal and external (commonwealth/state government) funding requirements
- Supervise and support staff that directly report to this role to ensure data is accurate
About you:
- Relevant tertiary qualifications in a health, education or related field; Project management qualifications and experience are desirable
- Demonstrated experience of successfully managing multi-stakeholder projects or programs, including managing project teams; experience with managing government-funded contracts and donor relationships are highly desirable
- Capacity to work both autonomously and in a self-managed team environment on a range of diverse issues to meet stringent project requirements
- A can‑do, helpful and positive approach to fast‑paced work with the ability to pivot and adapt to changing circumstances
- Demonstrated capability to review and ensure reporting requirements are accurate and timely to meet the requirements of the organisation or funders as relevant
- Strong understanding of the Australian primary health care system, including understanding of health care delivery systems and patient care approaches such as coordination of care
