Overview
Wurli-Wurlinjang Health Service (WWHS) is an Aboriginal Community Controlled Organisation that provides a range of health services to the community. WWHS operates clinics offering primary health care and services including the Main Clinic (acute and general care), Gudbinji Chronic Disease Clinic, Binjari Health Centre, Women and Children’s Health, Men’s Health and a Dental Clinic. The Wurli Community Services team aims to improve outcomes in early intervention, treatment and management of mental health and other social and emotional wellbeing concerns for Aboriginal and Torres Strait Islander people through an integrated primary health care approach. The Katherine Individual Support Program (KISP) operates within Wurli’s Community Services Department, alongside programs addressing addiction, domestic and family violence, Strongbala Justice, and SEWB. These services provide an integrated continuum of care that supports individuals, families, and the community to achieve better health, healing, and social outcomes. The KISP Coordinator supports outreach and case management for Aboriginal and Torres Strait Islander people experiencing homelessness, overcrowding, chronic health conditions, and complex social needs. Service delivery is grounded in cultural responsiveness, trauma-informed practice, and strengths-based approaches. The SEWB framework underpins practice across all program areas. Indigenous and/or Torres Strait Islander persons are encouraged to apply.
Responsibilities
- Support outreach and case management for Aboriginal and Torres Strait Islander people experiencing homelessness, overcrowding, chronic health conditions, and complex social needs.
- Contribute to service delivery that is culturally responsive, trauma-informed, and strengths-based.
- Engage in community awareness and capacity-building activities and build trusted relationships to advocate for systemic change.
- Collaborate with multidisciplinary teams and foster collaborative approaches across agencies and community settings.
- Utilize client management systems such as Communicare, with a commitment to workplace wellbeing and prevention of psychosocial hazards.
- Meet the requirements of current police checks, Working with Children’s Checks, and driver’s license requirements; travel within the Big Rivers Region may be required.
Qualifications
- Relevant tertiary qualification in Social Work, Psychology, Counselling, Nursing, Aboriginal and/or Torres Strait Islander Primary Health Care, Alcohol and Other Drugs, Community Development, or related discipline; or Certificate IV (or higher) as an Aboriginal and/or Torres Strait Islander Health Practitioner with substantial experience in case management, service coordination, or program leadership.
- Demonstrated experience in community development, program coordination, and service delivery in health or community service settings, preferably within Aboriginal and Torres Strait Islander communities.
- Proven ability to design, lead, and evaluate culturally safe, trauma-informed services for people with complex needs (e.g., chronic illness, mental health challenges, substance use, homelessness, and social disconnection).
- Ability to build trusted relationships, advocate for systemic change, and engage in community awareness and capacity-building activities.
- High-level communication, facilitation, and conflict resolution skills; competence in client data management, reporting, and compliance.
- Experience supervising multidisciplinary teams and fostering collaborative, strengths-based approaches across agencies and community settings.
- Proficiency in or the ability to quickly learn client management systems such as Communicare.
- Current National Police Records Check and Working with Children’s Clearance.
- Current Northern Territory Driver’s License and ability to travel within the Big Rivers Region.
