Are you a compassionate and skilled professional with a nursing degree looking to support individuals with disabilities and their families? We are seeking a dedicated Family Support Worker to provide essential in-home and community-based support for individuals with disabilities and their caregivers.

About the Role:
As a Family Support Worker, you will work closely with individuals with disabilities and their families, providing personal care, emotional support, and assistance with daily living activities. Your nursing expertise will play a key role in ensuring that individuals receive the highest standard of care, promoting independence, social inclusion, and well-being.

Key Responsibilities:
Provide personal care (bathing, dressing, grooming, toileting)
Assist with mobility and transfers, including the use of hoists or other aids
Support with meal preparation, household tasks, and medication reminders
Help individuals develop life skills and engage in community activities
Provide emotional support and companionship to individuals and families
Assist families in accessing NDIS and other disability support services
Support with transport to appointments, social outings, and daily activities
Use your nursing knowledge to monitor health conditions and provide care support
Maintain accurate and confidential care records and reports

Requirements:
Nursing Degree (RN or equivalent)
Certificate III or IV in Individual Support (Disability, Aged Care, or Community Services) or a related field
Minimum 12 months of experience in disability support, aged care, or community services
Experience working with individuals with physical, intellectual, or psychosocial disabilities
Current Working with Children Check and National Police Check (or willingness to obtain)
Current First Aid & CPR Certificate
Valid Driver's License and reliable vehicle (preferred)
Strong communication, empathy, and interpersonal skills
Ability to work with diverse individuals and families
