Right at Home Gosfordare seeking a qualified and experienced Care Coordinator for our home care agency. Our services include, aged and disability care, transitional care, hospital in the home and palliative care. Our priority is to help our clients live empowered and independent lives at home. Our mission is to improve the quality of life for those we serve.
Our preferred candidate will demonstrate client-centred practice, showing empathy, and treating others with dignity and respect. You’ll be a natural relationship builder, helpful, and reliable; making you a great asset to our team.
Your new role will be responsible for:
- Meeting with clients and their families, coordinating their needs within their funding mode
- Conducting client care reviews and actioning care plans to best meet client requirements
- Handling information requests, feedback, and general enquiries
- Scheduling services for our clients by rostering care workers dependent on their qualifications and availability
- Ensuring all documentation is accurately and comprehensively completed in Right at Home databases and software programs
Qualifications
To be successful in this role, you will need:
- Highly developed verbal and written communication skills
- Strong organisational, problem solving and time-management skills
- Current or willing to obtain Australian National Police Check & Working with Children Check
- Current or willing to obtain First Aid & CPR certificate
- Full Driver's license and your own reliable, registered vehicle
- The right to work in Australia
Preferred Skills
- Sound understanding of the Aged Care and Disability systems
- Demonstrated experience supporting staff
