You will be working alongside experienced, passionate leadership peers who are true to our values of I integrity, I innovation, C collaboration, A accountability, R respect and E empathy.

Make a real difference by supporting older people to live safely and independently at home. As a Clinical Care Partner, you'll lead the delivery of high‑quality, person‑centred clinical care under the Support at Home Program. You'll provide expert assessment, advice and education to clients, families and support workers, while identifying and managing clinical risks. From developing and reviewing care plans to coordinating multidisciplinary services and delivering hands‑on clinical care, you'll ensure seamless, responsive support that meets each individual's needs. As a trusted clinician and a team role‑model, you'll promote safety, dignity, and cultural respect, maintain compliance and quality standards and foster collaborative, positive workplace focused on continuous improvement.

You will bring:

- Bachelor of Nursing

- 3 years postgraduate experience

- Punctual, reliable and accountable.

- Excellent communication and organisational skills.

- Driven by a passion for providing high‑quality support to employees and clients alike.

The successful applicant will hold, or be able to obtain, a current National Criminal Police Check. Dependent on your position you may be required to obtain a Working with Children Check Clearance and / or NDIS Worker Screening Check. All successful applicants are to comply with WDHS immunisation and vaccination requirements.
