Case Manager – Aged Care

Bamboo Home Support is a registered NDIS and Aged Care provider, committed to delivering person-centred care across multicultural communities in Northern Adelaide and surrounding areas. We aim to empower our clients through responsive support and holistic care planning.
Role Overview:
We are seeking a passionate and experienced Case Manager to oversee client care, coordinate services, and ensure compliance with care plans under the NDIS and Support at Home program. This role is critical in ensuring client outcomes are met through effective planning, communication, and service delivery oversight.
Key Responsibilities:
- Develop, review, and implement individualised care plans
- Conduct assessments and monitor client progress
- Coordinate with support workers, allied health, and families
- Ensure accurate documentation, progress notes, and incident follow-ups
- Monitor risk factors and implement mitigation strategies
- Liaise with external stakeholders and funding bodies
- Maintain compliance with NDIS and aged care regulations
- Submit monthly performance reports to senior management
Mandatory Requirements:
- Qualification in Nursing, Community Services, Case Management, or equivalent
- Strong understanding of NDIS and/or Aged Care standards
- Experience in support coordination, case management, or care planning
- Current NDIS Worker Screening Check
- National Police Clearance (recent)
- Current First Aid and CPR
- Humanization History & Infection Control Training
- Unrestricted SA Driver’s Licence and own vehicle
- Strong communication and interpersonal skills
Desirable:
- Experience with CALD communities
- Background in aged care, disability support, or community nursing
- Familiarity with BambooCare software (or similar care management tools)
