Central Coast Local Health District have a great opportunity for you to bring your expertise to the forefront with new positions available within the Older Persons Mental Health Team in Wyong**
- Develop your skills working in an environment which is recovery-oriented and attentive to the patients biopsychosocial needs
- Demonstrate your expertise working in partnership with consumers, carers, general practitioners and other services and supports in support of our older community
- Play a part in supporting patients early discharge and assist with their discharge plans facilitating their healthy return to community

**About You**
- You have experience working with consumers and carers over the age of 65 years and a have a demonstrated understanding of the issues for older people experiencing moderate to severe mental health issues
- You hold a Tertiary Bachelor Degree in Social Work (where degree provides eligibility for membership of the Australian Association of Social Work) or a Degree in Occupational Therapy (plus general registration with the Occupational Therapy Board of Australia) or Nursing with Division 1 Registration as a Registered Nurse with AHPRA a Degree in Psychology with Registration with the Psychology Board APHRA
- You hold a current valid Australian Drivers Licence with a willingness to travel within Central Coast geographical locations
- You have an understanding of recovery models in mental health and are driven by ethos of implementing these models within clinical practice.

**The position of Older Peoples Mental Health Care Coordinator is suitable for the following**:

- Social Worker Level 1/2
- Occupational Therapist Level 1/2
- Registered Nurse
- Clinical Nurse Specialist Grade 1
- Psychologist
- Clinical Psychologist

You will be working within a multidisciplinary Team of specialised and dedicated individuals who provide community mental health services to persons over the age of 65 years living within the community setting (including residential aged care facilities) on the Central Coast.
