**The Opportunity**
The **Care Coordinator** will be part of our amazing Amplar Preventative Health Team. In this role you will be coordinating clinical support to participants who may have recent hospital admission and/or complex chronic illnesses. This role will be a mix of supporting a caseload of participants (managed virtually, i.e. work from home) and visiting new participants in their homes.
Drawing on your holistic knowledge and background in health, you will assess and mitigate patient risks by understanding their key motivators for change. You will utilise a coordinated and integrated approach, facilitating health behaviour change and care coordination in the delivery of health coaching, injury and illness recovery, and chronic and complex disease management programs.
This is a 12-month fixed term contract role with **part time or full time** (0.6-1.0 FTE) options. We currently require clinicians to be located in **Melbourne, VIC** to cover Airport West or Brunswick areas, **Sydney** and **Brisbane**.
Please note this role is a non-hands on, clinical role.
**About You**
You are an advocate for preventative health and have the natural ability to build rapport with a wide range of clients and healthcare professionals. You have previous experience working in the community environment across a diverse demographic and a solid understanding of Chronic Disease management is essential for this role.
Previous experience in care coordination or case management will be highly regarded. As well as exposure to clients with mental health issues, and an awareness of mental health services.
Working in this highly autonomous position you will have an organised and flexible mindset that allows you to meet the unique needs of your clients.
To be considered for this role you will be a **Registered Nurse** or **Allied health Professional (Occupational Therapist, Physiotherapist, Dietician or Exercise Physiologist) **with relevant registration with AHPRA, ESSA or DAA.
As this role requires you to make home visits a driver’s license and car are essential. You will also be working from home so will need to have a suitable office area from which to work and reliable internet connection. As this role involves face to face interactions with clients full **Covid 19 vaccination is required**.
