**Job description**:
DFP Recruitment is seeking several Clinical Complex Case Managers to join our client. We have over 10 positions in Brisbane, Sydney, Adelaide, Melbourne, Perth and Canberra.

This Federal Government Department delivers government programs to support Military Veterans and their families. They are currently seeking experienced Case Managers (Social Worker, Mental Health Registered Nurse, Psychologist, and Occupational Therapist) to join their Triage and Connect Intake Team in a full-time capacity on an initial 12 month contract.

Triage & Connect provides an access point for Military Veterans and their families to services and supports available through this Government Department. The Team provides short-term case management to clients who may present with mental health acuity and associated risk of harm to self and others, as well as a range of psychosocial needs. Referrals to the Team come from internal and external stakeholders and all case management support is delivered by telephone.

**The role**

You will be responsible for the assessment and risk management of clients referred for support in relation to mental illness, unmet psychosocial needs and challenges navigating the Departmental processes. You will work as a member of an established national team of clinical and non-clinical staff and will be responsible for:

- Providing short-term, evidence based and trauma-informed clinical case management to a caseload of clients identified with complex health and/or multiple psychosocial needs.
- Engaging with clients utilising a clinical lens in order to assess needs and risk and to establish responsive case plans which link clients to identified services and supports.
- Assessing and responding to risk on an ongoing basis. This may include escalating for emergency support when necessary, in line with Departmental protocols.
- Liaising with internal and external stakeholders in order to develop strong support networks for clients.
- Maintaining case records according to Departmental standards and collating and analysing data from client records.

On a rotating basis you will also be rostered on as Duty Officer for the Team. The Duty Officer role responds to urgent client needs, particularly risk of harm to self and others. You will also be rostered to assist with the Team’s phone.

**To be successful you must have the following**:

- Qualification as a Social Worker, Psychologist, Mental Health Registered Nurse, or Occupational Therapist with registration (or eligibility) with the relevant professional body.
- Frontline experience in assessing and responding to clients with complex mental health presentations, including acuity and risk of harm to self or others.
- Experience in short-term case management of a case load of clients with complex health and/or multiple psychosocial needs via assessment, case planning, and referral to appropriate supports.
- Strong time management skills, flexibility, and responsiveness in a fast paced environment and the ability to work independently.
- Demonstrated IT skills and the ability to navigate multiple IT systems.
