Grade 2 Social Worker position - Fulltime or Part-time
- 3-6 month locum position with possible extension
- $55 - $60 p/h + super + Free accommodation.

On behalf of our client which is a well established Health Service, we are seeking an experienced Hospital Social Worker to join the Allied Health Team who provide high quality social work and care coordination services to clients and relevant support to carer and family members in relation to the client’s wellbeing and access to services. The Service have the latest technologies allowing our 5000 staff to deliver excellent care to their community. The Allied Health teams provide a diverse range of programs and person centred care in inpatient, outpatient, community, home and residential care settings. This position can be full time or part time for 3-6 months with possible extension.

**Key Responsibilities**
- Maintain timely, effective and professional oral and written communication with external professional and organisations as required.
- Work within the multi-disciplinary team, recognising and respecting the expertise and contribution of all team members.
- Service Planning and Co-ordination whilst providing excellent, evidence based clinical care to patients and families/carers.
- Present a broad range of complex psychosocial needs including:
Comprehensive /Psychosocial assessment, Advocacy, Referral, Interventions for at risk populations, Support and counselling for patients and families in crisis situations including severe trauma, family violence and bereavement.
- Develop, document and implement individual client care plans with other health professionals including GPs to address issues with clients/carers Information Management.
- Ensure information is managed in line with organisational and discipline standards.
- Complete reporting and statistical records within the specified timeframes as required.

**About You**
- Qualification in Social Work
- Current Membership with AASW or eligibility to obtain
- High level of communication, interpersonal and negotiation skills.
- Capacity to provide a range of high standard interventions including individual and family counselling, crisis intervention, short term and supportive counselling, group work, education, advocacy and health promotion as appropriate to the needs of the patient and/or family.
- Experience in care needs assessments, care planning and care coordination for people with complex and chronic conditions.
- Demonstrated experience and skills in the area of health focused psychosocial assessments, risk assessment and safety planning.
- Knowledge and understanding of the underlying conditions associated with ageing, frailty and disability.
- Demonstrated commitment to ongoing professional development.
- Demonstrated time management skills to manage a case load and set goals / objectives

**Salary**
Casual Agency Rates of $55 - $65 p/h + super + Free accommodation.
