**About the role**

Our Intensive Family Support program offers voluntary in home family support to families who are at risk of entering or re - entering the Child Protection System. Through case management and effective relationship building, we aim to strengthen families so children can live at home safely, free from abuse and neglect.

As a Case Manager (Child & Family Practitioner), you will engage and provide intensive support to at risk families to help them make positive choices in their parenting. Using your practice framework as well as Structured Decision Making tools, you will undertake an initial assessment of a family’s strengths and needs, and develop case plans that build on the family’s resilience to enable them to overcome risks and increase the family’s functioning.

**Duties**
- Work closely with statutory child protection services and other statutory and community services to increase safety and family functioning within the home.
- Develop an understanding of the primary needs of the family including consistent assessment of risk and needs using the Common Assessment Tools determined by the Department.
- Engage clients including children, young people, adults and/or families, by working closely with them during the assessment process and utilise this information to tailor specific in home support aimed at increasing capacity as well as reducing risk.
- Work with clients to develop, implement, monitor and review their client plan focusing on outcomes that identify strengths as well as areas of risk, ensuring children and young people are actively engaged in the process
- Coordinate with other service providers to advocate, negotiate and assist clients to access including government departments and agencies, schools, health services, GP’s and allied health providers to ensure services are delivered
- Work in a dynamic team that encompasses, Domestic and Family Violence Specialists, Identified Child and Family Practitioners, Youth Child and Family Practitioner and a Parenting group facilitator to deliver a comprehensive support service, ensuring that the child’s safety is paramount.

**Skills and Experience**

We want you to be successful in your new role This means you’ll have a Bachelor’s Degree in Social Work or Human Services field, and:

- Two years relevant experience (desirable but not essential)
- Excellent understanding of child protection issues and client vulnerabilities such as drug and alcohol use, domestic violence, mental health issues, and the impact of trauma and the effect on childhood behaviour and development
