Why this role is important

We are looking for a passionate, relationship focused Case Manager to join our Youth stream This is an incredible opportunity to be part of a dynamic team that has a client focus whilst supporting each other as a team.

In this role you will provide responsive case management to young people between the ages of 15 and 25 years of age, while effectively building relationships with clients and encouraging self-determination and participation. As well as that, you will be supporting the development and growth of young people's lives through the collaboration with internal and external stakeholders.

You will successfully- Support young people in line with Youth Services model of care and associated Frameworks- Build effective relationships with clients that encourage self-determination and participation- Provide ongoing assessment of client's issues, safety, needs and circumstances from initial intake to exit- Develop individual case plans in collaboration with clients including a range of interventions that address the clients needs- Assist clients to access support services and establish community supports including referrals to appropriate specialised agencies- Maintain accurate up to date client records including case notes using TSA information systems

This is full time permanent position, located in Ingle Farm, SA. Salary and conditions are in accordance with the SCHADS Level 5 Award.

You will have- Degree qualification in social work, welfare, community development or related field- Demonstrated high level of sector knowledge and skills attained through previous roles, service and study/development- A valid Employee Working with Children Check- Psychological Assessment (in line with Children and Young People Safety Act 2017- Through their Eyes (Reporting Abuse and Neglect) Training- A First Aid Certificate- A willingness to undergo a National Police check
