ABOUT THE ROLE
Reporting to the Offender Services and Integration Manager, the Assessment and Transition Coordinator undertakes prisoner risk assessments using the approved Corrections Victoria Reintegration Pathway, and provides a range of services to assist prisoners to transition into, through and out of prison. This includes:

- Participating in the Case Management Review Meetings to develop and review progress against plans.
- Coordinating prisoner involvement in transitional programs.
- Liaison and provision of assistance to external service providers.
- Facilitating referrals and coordinating the delivery of the programs.
- Liaison with internal and external stakeholders in relation to release and facilitation of information.

RESPONSIBILITIES

**The core responsibilities of this role are to**:

- Undertake prisoner risk assessments and needs assessments, and contribute to the case management process.
- Support the delivery of the Corrections Victoria Reintegration Program Pathway at Port Phillip Prison.
- Liaise with prisoners and prison staff regarding the outcome of risk assessments, and the delivery of transition and reintegration programs and services provided at the prison.
- Liaise and consult with contracted providers delivering pre and post-release services, refer individual prisoners to Corrections Victoria Reintegration Pathway Programs and/or Services, according to eligibility requirements.
- Contribute to and inform processes regarding location-specific needs in delivering reintegration services.

**The essential requirements from you**:

- Ability to work both independently and as part of a team.
- Ability to maintain proficient record-keeping and case file notes.
- Proven Experience in a position that required stakeholder engagement and relationship management.
- Commitment to continuous improvement and quality customer-focused service delivery.
- Previous experience in working in a correctional environment would be beneficial but not essential.

A current Driver's Licence is needed for this position.
