**About Kingston Aged Care Assessment Service**:
The objective of the Kingston Aged Care Assessment Service (ACAS) is to provide a comprehensive, holistic assessment of an older person or a younger person with an age-related needs. The assessment is to identify the resources, strengths and problems of the person and to develop an individualised, coordinated support plan which focuses on client goals and re-ablement. ACAS assesses a person’s eligibility for Commonwealth funded care such as home care packages, restorative care, transition care, respite and residential care

**About The Role**

The ACAS assessor position is designed to provide a comprehensive, holistic assessment of an older person or a younger person with an age related needs. The assessment is to identify the resources, strengths and problems of the person and to develop an individualised, coordinated support plan which focuses on client goals and re-ablement.

All staff need to ensure the operational requirements of the program remain in line with the Commonwealth, the Victorian Department of Health and Human Services and Monash Health guidelines. This position is based at Kingston Centre, and entails travelling to home assessments within the Cities of Kingston, Bayside and Greater Dandenong as well as private hospitals in the area and Monash Health inpatient sites.

**About You**

Exceptional and effective communication skills with great attention to detail. Able to work collaboratively within a team environment while also being able to work autonomously. Excellent knowledge of the healthcare, aged care and community setting and advanced clinical assessment skills.

**What You Need**:

- Relevant Qualification (Registered Nurse or Allied Health)
- Demonstrated experience in Nursing or Allied Health
- Current AHPRA registration (if applicable to profession)
- Current Australian driving licenses
- Australian working rights
