**PROGRAM/POSITION OVERVIEW**
The Support Coordinator is responsible for supporting NDIS participants in achieving their individual goals through coordination of support services. The position will support the development and implementation of NDIS participant plans, so as to maximise positive outcomes and achievement of individual goals for each client. The role strengthens outcomes for both the client and families through integrated and coordinated service delivery.
**MAIN DUTIES**
- Work with NDIS participants to understand their individual needs, goals, and preferences. Based on this information, develop a plan to coordinate the necessary supports and services to help participants achieve their goals.
- Manage the allocated budget for each participant's NDIS plan. Ensure that the supports and services provided are within the budget limits while effectively meeting the participant's needs and goals.
- Continuously monitor the progress of participants' NDIS plans. Track the outcomes and goals outlined in the plans to ensure that the supports are effective and that the participant is making progress.
- Collaborate closely with participants to understand their broader needs and goals beyond just the support services. Take a holistic approach to address various aspects of their lives, such as social, emotional, and personal development.
- Maintain accurate and up-to-date records of all interactions, services provided, progress updates, and relevant documentation. This is essential for compliance with NDIS regulations and for reporting purposes.
- Keep track of billable hours for each participant. This involves recording the time spent on coordinating services, meetings, and other activities related to the participant's support plan.
- Establish and maintain proactive relationships with both NDIS registered and non-registered service providers. This enables you to connect participants with appropriate services and supports that align with their goals.
- Collect and coordinate any evidence required for NDIS plan reviews. This might involve documenting progress, gathering feedback, and updating goals to reflect the participant's changing needs.
- Contribute to the growth and development of the support coordination service. This could involve identifying areas for improvement, suggesting new strategies, and participating in team discussions to enhance the effectiveness of the role.
**ESSENTIAL**
- Tertiary qualifications in Social Work, Psychology or related field, or experience deemed to be the equivalent.
- Strong experience in the development and delivery of sustainable outcomes for people with an intellectual and developmental disability as well including from a CALD background.
- Knowledge and understanding of disability sector funding including NDIS.
- Ability to engage and manage complex issues related to servicing Culturally and Linguistically Diverse clients.
- Demonstrated superior skills in mediation and negotiation, collaboration and consultation with key stakeholders.
- High-level knowledge and understanding of the policy, procedures and standards pertaining to the provision of disability services.
- Commitment to social justice principles and access and equity.
- Demonstrated high level customer/sales and networking/marketing service skills.
- Willingness to learn and drive the growth of support coordination at Woodville.
