**About the Opportunity**:
MDS is seeking to employ a **6 months** **Fixed Term Support Coordinator** for our Support Coordination team. This role supports people with a National Disability Insurance Scheme (NDIS) plan to connect to their communities and access a range of supports across one or more providers.** **This program is experiencing a significant amount of growth and MDS is seeking additional Support Coordinators with Disability and/or Mental Health Experience to join our team.

Key Responsibilities for this position include:

- Provide varying degrees of coordination and transition supports for children, young people and adults with a disability
- Support implementation and identify options for all supports in the plan, including informal, mainstream and community, as well as funded supports
- Strengthen and enhance the participant's abilities to coordinate supports and participate in the community, reach decisions and develop agreements with support providers
- Work within the funding levels allocated for each person and monitor and track billable hours
- Managing a high volume caseload

The successful applicant will have:

- Certificate IV or higher in Community Services/Disability Work/Case Management OR relevant demonstrated knowledge and experience
- Knowledge of the National Disability Insurance Scheme (NDIS) and the disability sector
- High professionalism and experience working collaboratively with multiple stakeholders in formal and informal partnerships to achieve outcomes.
- High standard and ability to articulate information, factual and evidence based written reports
- Ability to meet deadlines and manage competing priorities
- NSW Drivers Licence
- NDIS Worker Clearance
- Working With Children Clearance
- Current Covid-19 vaccination

Desirable:

- Previous experience working in a Support Coordination/Case Management/Care Coordination role
- Previous experience working with psychosocial/mental health support services or personal/lived experience in mental health

Position is Fixed Term due to a parental leave vacancy from 18/3/2024 - 30/08/2024. Working Part Time 66 hours per fortnight on on 8-10 day fortnight. MDS uses a hybrid working model, allowing staff to work remotely several days up to 2 days per week. This role is covered under the SCHCADS Award Level 4 classification ($41.52 - $44.68 per hour).
