**What you’ll be doing in this role?**

The Project Officer - Chronic Disease Management (CDM), will be the lead in coordinating the assigned and co-assigned LCHS Chronic Disease Action Plan items.

**Your work will include**:

- Identifying opportunities to improve and /or increase chronic disease management
support to our local community underpinned by innovation
- Expand on existing LCHS chronic disease projects and services
- Working with LCHS clinical staff to improve services, in line with the outcomes of the
action plan

**What we’re looking for**:

- A current Victorian Drivers Licence
- A current Working with Children Check (Employee) or VIT Registration
