**What you will be doing**:
An Exciting opportunity is available to join the Regional Assessment Service team as a Home Support Assessor. Home Support Assessors provide basic home-based face-to-face assessments of clients referred via the My Aged Care Gateway using the National Screening and Assessment Form (NSAF). Working with each client, the Home Support Assessor establishes goals specific to the client; develops appropriate strategies and actions, including referral to age care and non-aged care service; and develops a support plan. The Home Support Assessor completes the NSAF and matches and refers clients to the appropriate service(s).

Selection Criteria to be Addressed:

- Certificate III in Home Care or Aged Care or equivalent work experience.
- Statement of Attainment 2 and Self -Paced Learning Experiences x 3 (SPLEs) or willingness to undertake when commencing the role.
- Well-developed written, verbal and interpersonal communication skills including accurate spelling, good verbal skills and comprehension and ability to deal courteously with people at all levels.
- Demonstrated computer skills with capacity to learn and use electronic reporting systems and registration system.
- Demonstrated ability to work independently as well as effectively in a collaborative team environment.
- Ability to follow guidelines relevant to position as well as demonstrated problem solving capacity.
- Current Driver's Licence and willingness to use for work purpose travel.
