About the Role
The purpose of this role is to deliver Dietetic services within Chronic Disease Brisbane South and Chronic Disease Logan
**This includes**:
- Detailed clinical assessment, patient education, therapeutic intervention and/or management and assistance with discharge planning within a multi-disciplinary context.
- Clinical education, supervision of observational students and allied health assistants, and assistance with supervision of undergraduate students.
- Participation and contribution to quality improvement and research activities under the guidance of senior staff.
- Services include, but are not limited to, cardiac rehabilitation, diabetes, respiratory and Motor Neurone Disease and are delivered across multiple sites of Metro South Hospital and Health Service.
- The Chronic Disease Brisbane South and Chronic Disease Logan Team is a multidisciplinary, multisite team with a core focus that supports hospital avoidance strategies, to ensure clients remain as well as possible at home for as long as possible.
