We are excited to announce that we have a newly created **Care Manager** (Grade 5 RN) role at our **Warralily Gardens** Home located in **Armstrong Creek**.
- Located on the doorstep of the beautiful _**_Surf Coast_**_, _**_Warralily Gardens_** _sets a new standard for residential aged care. _
- Central to the vibrant local community, Warralily Gardens offers the most extensive range of aged care facilities and services in the _**_Geelong _**_region with _**_180 beds_**_._

You will thrive being on the floor with a high performing team in this hands-on role to provide our residents with exceptional care while maintaining positive relationships that contribute to a safe and caring environment.

**What you’ll be doing**
- Contribute to continuous clinical improvement to delivery excellence in care
- Support residents, families and visitors in a highly professional manner
- Ensure assessments, care plans and reviews are undertaken appropriately, on time and well documented
- Manage clinical education and competency of staff
- Promote and foster a positive, collaborative and diverse culture

**Hours of Work**
- Monday to Friday
- 0800 - 1615 (or variation depending on operational needs)

**Things that matter to us**
- Two years’ experience in a clinical leadership role
- Understanding of the Aged Care Quality Standards
- Relevant Nursing Qualifications (Division 1)
- Current registration with AHPRA
- Up-to-date COVID-19 vaccinations (3 doses)
- 2024 influenza vaccination
- NDIS Check (includes Police Clearance)
- Eligibility to work in Australia
