About You

**Additional attributes we are looking for include**:
Previous experience in assertive outreach, youth work, case management, mental health, justice or AOD (professional, volunteer, lived experience); or qualifications in psychology, mental health, social work or other relevant fields
The ability to effectively and quickly build rapport with people and think outside-the-box to achieve positive outcomes
Flexibility, patience, confidence, initiative, street-smarts and emotional resilience
Effective listening and negotiation skills & a strong but adaptable communication style with people of diverse backgrounds and abilities
The ability to contribute positively within a team and work autonomously to manage your case load
Demonstrated capacity to work flexibly, possess the ability to manage competing demands, and able to effectively set boundaries and limits where required.

It is a requirement of this position to be fully vaccinated against COVID-19 and meet all compliance requirements in relation to NDIS Work Screening, Working With Children’s Check, work rights and possess a current & valid driver’s license.

About the role
The Assertive Outreach Service (AOS) is a brand-new program focused on delivering a targeted, time-limited-service response to a small number of clients with complex functional needs, who experience significant service gaps and present an unacceptable risk of harm to others and/or self. The AOS program will be piloted initially for up to 12 months and will deliver assertive outreach and case management services to clients referred by the Department of Families and Fairness (DFFH) across the North West Metropolitan Region of Melbourne. Within this new program, the Assertive Outreach Support Facilitator will support people to overcome barriers to accessing services and is provided flexibility in the place of residence or community, to build capacity for people with complex needs and improve their engagement with targeted interventions and services. Working under the leadership of the Complex Case Manager, the Assertive Outreach Worker will deliver practical support allowing clients to obtain their desired goals.

**The Assertive Outreach Support Facilitator role entails**:
Provide assertive outreach and psychosocial support to young people and adults who are experiencing service gaps
Work with participants, clinical teams and other stakeholders (e.g. family) to access and engage with a range of supports
Develop, monitor and track recovery plans whilst encouraging engagement with support networks for individuals experiencing severe mental illness, crisis and/or homelessness
Complete assessments and participate in case plans in collaboration with care teams and liaising with clinical staff/other services
Advocate for others and build support networks whilst managing risk and resolving conflict

This role works on a hybrid system, including working from home and assertive outreach support to participants across Melbourne, primarily across the North West Metro Region. This role will require you to be willing to travel to unplanned locations at short notice.

Program hours are typically 8am to 8pm with potential for weekend work. This is a contract position ending 28/06/2024.
