**About The Opportunity**
The SWSLHD Aboriginal Chronic Care Program (ACCP) supports Aboriginal people and their families living within chronic conditions navigate the health system and build capacity for self-management. The program is unique because of its integrated model that involves engagement across the tertiary and primary care sectors and collaboration with Aboriginal Community Controlled Organisations.

The ACCP Manager is responsible for leading this innovative service and its large multidisciplinary team. The position is responsible for the development and maintenance of service networks that support Aboriginal people with chronic conditions to access timely culturally responsive care.

The role requires an Aboriginal person with a commitment to embedding culture in practice and experience managing 4 multidisciplinary teams that support clients with complex needs.

**About the Team**
The ACCP has a large multidisciplinary team comprising of Aboriginal Health Workers, Nurses and Allied Health professionals. The service has a leadership team that includes a number of senior clinical roles who are responsible for ensuring the service maintains its cultural integrity and strong clinical governance. As part of the unique service model ACCP operates two Aboriginal Community Health Centres in Miller and Bankstown and provides outreach across the South Western Sydney region using a home visiting model.

**Where you’ll be working**:
SWSLHD operates its services on the lands of the Dharawal, Darug and Gundungurra lands and has a rich history of working alongside local Aboriginal communities. SWSLHD has long established partnerships with Aboriginal Organisations in our region that are critical to the way we operate and deliver our services.

The ACCP Team is based within Primary and Community Health and operates from Miller but is networked in as part of the larger SWSLHD Aboriginal Health Service. The ACCP Manager is part of the Aboriginal Health Leadership Team that includes Aboriginal Managers from across a range of services. You will have access to established supervision structures including clinical supervision, cultural supervision, and peer supervision. We offer a wide range of cultural supports across the service including mentoring from experienced Aboriginal Leaders, training and development opportunities as well as a large workforce of Aboriginal colleagues.

- This is an Aboriginal Identified role. Describe how your connection to culture has influenced your understanding of the Aboriginal chronic disease prevention and management policy frameworks.
- Tertiary Qualifications or work experience related to establishing systems that ensure the cultural integrity and effective clinical governance of health services or programs.
- Experience in directly managing an Aboriginal health service or program with a multidisciplinary team.
- Demonstrated experience in establishing systems that ensure the cultural integrity and effective clinical governance of health services or programs
- Demonstrated experience in planning, design, implementation and evaluation of health services within an evidence based framework
- Demonstrated experience in developing service networks and collaborative service models that improve access to health services for Aboriginal communities.
- Demonstrated advanced communication skills (verbal and written) and analytical skills, including the ability to lead complex negotiations and decision making processes.
