- to clients with complex health needs in a community setting- Highly developed communication skills including negotiation, conflict resolution, supporting

Quality Service - Support program development and growth by educating and informing
potential clients and relevant stakeholders of Mayflower’s Home Care
Services
- Provide regular clinical and operational supervision to Case Managers
encouraging exceptional service delivery and adapting work practices to
address significant changes in the sector
- Manage rostering with direct reports
- Review and develop processes relevant to case management that ensure
HCP and program services, legislation, clinical standards, client safety and
program target are achieved

- Identify and mitigate risk with early prevention strategies
- Ensure the case management team recognise, respond, and escalate
- changes to client’ conditions and manage them accordingly- Exercise judgement within the framework of operational and budgetary

stakeholders such as subcontracted providers, referrers, and discharge
